We are committed to protecting your privacy. This Privacy Policy lets you know
how your personal information is processed and used. We do not collect personal
information from you unless you voluntarily and knowingly provide it to us. We
do not provide or sell information about our members or site visitors to
vendors.
This Privacy Statement applies to the Cerebral Palsy Register site. The
Register is not responsible for the privacy practices, privacy statements, or
content of various other web sites linked to ours.

Information We Gather from You
Personal Information
We do not collect any personal information from a visitor to our site unless
that visitor explicitly and intentionally provides it. If you are simply
browsing our site, we do not gather any personal information about you.
There is only one way in which you may explicitly and intentionally provide the
Register with and consent to our collection of certain personal and health

Your Decision to Become a Member of the Register
If you choose to become a Member of the Register, we will collect your name and
contact details for the purpose of issuing a Username and Password. We may also
contact you from time to time with updates on issues relevant to cerebral palsy
research. If you would like to opt-out of being contacted, please contact us at
cpregister@tscnsw.org.au
Your Decision to Contribute a Cerebral Palsy Record to the Register
If you choose to notify us of your/your child's/your patient's Cerebral Palsy
Record we will also require written, informed consent.
Completion of a Cerebral Palsy Record requires you to provide personal,
parental and health details to ensure a complete population database. Where
practicable, the Register will collect this information directly from the
individual concerned. Alternatively we will collect it from a third party such
as a parent, health professional or hospital.
The Register collects personal information in a number of ways, including:
Directly from you, when you provide information via this web site or by phone
or in documents such as a Membership or Register Record form
From third parties such as parents, health professionals and hospitals via this
web site or by phone or in written documents
From publicly available sources of information
From our own records of how you use our services.
If you enter information in the process of completion of an on-line Register
Record form, the information will be collected by us only if you "submit" that
on-line record to us. If you log out prior to submitting your record, any
information that you have entered will be automatically deleted. If you prefer,
the Cerebral Palsy Record forms can be printed for written completion and then
mailed to the Register.
Statistical Information About Your Visit
The Register logs all activity and maintains audit trails of usage of this
site. We use this information in the detection and prevention of fraud or other
malicious activities.
When you visit our site, our computers may automatically collect statistics
about your visit. This information does not identify you personally, but rather
about a visit to our site. We may monitor statistics such as how many people
visit our site, the user's IP address, which pages people visit, from which
domains our visitors come and which browsers people use. We use these
statistics about your visit for aggregation purposes only. These statistics are
used to help us improve the performance of our Web site.

Use and Sharing of information that you provide to the Register
The Register will use and disclose data it holds about an individual for the
purpose of providing information about cerebral palsy.
Information will mainly be provided via the Fast Facts reports that can be
accessed from this web site. Only aggregate information is provided via the
Fast Facts reports, no personal or identifiable information is included in
these reports.
The Register will compile and publicise an Annual Report every year. This
report will contain aggregate information about people with cerebral palsy, no
personal or identifiable information is included in these reports.
We will use personal information to reply to inquiries, handle complaints,
provide operational notices and in removing duplicate entries from the
Register.
Personal information will remain confidential unless:
You provide us with written or electronic consent
We are required by law to make the disclosure.
Register Record Linkage
A unique feature of this web site is the option to link your Register Record
with your nominated health professional. This means the Register will issue
your nominated health professional with a Username and Password that will allow
them to view your record and complete any outstanding clinical details. You may
ask the Register to remove/add linkages at any time.
Access and Correction
Generally, the Register allows individuals access to their own information
through the secure part of this web site. Using their Username and Password an
individual may review and update their membership details and/or Register
Record at any time.
In addition, the Register will correct information it has about an individual
if it discovers, or the individual is able to show, the information is
incorrect. If an individual seeks a correction and the Register disagrees that
the information is incorrect, the Register will provide the individual with
it's reasons for taking that view.
Participation in Surveys and Research
From time to time we may ask members and people on the Register to participate
in a voluntary survey. Survey information will be used for research purposes or
to improve this web site and our customer service.
In addition we may contact members and/or people on the Register to inform them
of opportunities for participation in voluntary medical and health research. If
you would like to opt-out of being contacted, please ensure your tick the
correct box on the Register Record Consent Form or contact us at
cpregister@tscnsw.org.au
Our Security Procedures
To ensure security, the Register uses a highly secure internet connection
(known as Secure Sockets Layer) to protect against the loss, misuse, and
alteration of any information we receive from you. As with any transmission
over the Internet, however, there is always some element of risk involved in
sending personal information.
Popular browsers such as Netscape Navigator and Microsoft Internet Explorer
will alert you when information is being protected by SSL.
Changes to this Privacy Statement
If a material change is made to this Privacy Statement and/or the way we use
our members and Register Records personally identifiable information then, we
will post prominent notice of the nature of such change on the first page of
this Privacy Statement and also on our home page.
